How It Works
Taking the first step can feel overwhelming. We've made our process as gentle and reassuring as possible.
1. Book a Consultation Call
It starts with a simple, no-obligation chat. We want to hear about your worries, understand what's changing for you or your loved one, and see if preventative care is the right fit.
2. The Five Pillars Assessment
If we agree to move forward, we visit you or your loved one at home. Rather than focusing only on what's difficult, we assess strengths and vulnerabilities across our Five Pillars: Physical, Cognitive, Nutrition, Social, and Home Safety.
3. Your Personalised Plan
We don't do off-the-shelf care. We design a specific plan focused on maintaining independence. This might mean daily walks to build leg strength, or cooking together to ensure good nutrition while staying active.
4. Staying Stronger Together
Our carefully matched care team begins their visits. Crucially, we constantly monitor progress. As you or your loved one gets stronger, we adapt the plan. We keep you updated every step of the way, giving you your peace of mind back.
Ready to take the first step?
There's no commitment and no pressure. Just an honest conversation about how we can help you or your parent stay independent.
Book Your CallA framework for living well at home, not just getting by in it.
Every Abney & Baker care plan is built around these five pillars — how we measure success, how we work with you to structure your care and support, and how we know when someone is genuinely thriving, not simply surviving.
Physical health
Improving strength, balance and mobility to reduce the risk of falls and injuries.
Cognitive health
Keeping the mind active and engaged to protect memory and mental agility.
Nutrition
Supporting healthy eating and hydration to boost energy and wellbeing.
Social & emotional wellbeing
